Do poor 3rd world folks really "choose" to work in these jobs? Are you sure that they wouldn't accept experimentation--especially given that it is likely to be perceived as better than any real "treatment" they'd get? I am on the side of bioethicists who say "standard of care" should be based on the country sponsoring the clinical trial. If a drug company could not/would not conduct the same research protocol in suburban Maryland, then it is not OK to conduct it in rural Alabama or Nigeria or on minority foster kids in NYC. But it's also necessary for us to recognize how we are implicated in other such exploitation of developing nations. We Americans like safe drugs and fly athletic gear. It's only "simple" if you're willing to cast Big Pharma as the perpetual bad guy instead of recognizing our own role in these types of situations. |
